The terms and conditions of the draw are provided below. Winner of the lottery draw will be contacted by phone and in writing (by email or letter), and his/her name (first initial and surname only) will be posted on the University of Queensland’s School of Public Health website (

Prize Draw Terms and Conditions


  1. Participants who submit a completed survey are eligible to enter the draw;
  2. Entry into the draw requires a participant to:
  • a. complete and submit the online "Tobacco and Vaping Policy Survey"
  • b. consent to providing his/her personal contact details (title, full name, address, email, phone number)
  • c. provide valid personal contact details (title, full name, address, email, phone number) for the purpose of receiving notification if he/she wins the draw, and
  • d. fulfil participant inclusion criteria as detailed in the study information.


  1. One prize valued at $1000 in the form of a shopping gift card
  2. Only one prize is available to be won.
  3. Only one entry per person
  4. The draw to select the winning participant will be performed by a computer generated random selection process.
  5. The prize will be drawn on 28th October, 2016, and the winner will be notified within 7 days.
    Note: If data collection has not been finalised by this date, the date of the prize draw will be extended at the discretion of the research team.
  6. The winner will be notified by phone and in writing (either by email or letter) using the contact details provided to the research team.
  7. The winner’s name will be published on the University of Queensland's, School of Public Health website within 7 days of the draw.
  8. The shopping gift card will be mailed to the draw winner.

Entrants into the Prize Draw shall be deemed to have accepted these Terms and Conditions.

If a winner cannot be directly reached and does not respond within 14 days of being notified, the prize will be forfeited and the research team is entitled to select another winner.

The prize is non-exchangeable, non-transferable, and is not redeemable for other prizes.
